If I take a ferry from Koh Lipe to Langkawi, then fly to Penang and back to Thailand will that get me a new 45 days?
 

Or do you have to depart and return from airports?

Yes, that resets it.

No need to go in and out of airports, as long as you see immigration at the border crossing.
